On 1/1/2007 Anthem introduced a new 'incentive' series of plans called Lumenos for those who'd like coverage but have no burning need for regular office co-pays or medicines. What we've quoted is the 100% pay plan -- meaning, if something catastrophic happens and you meet the $2500 deductible, the plan pays the rest. The Lumenos plans offer incentives plus quarterly deposits in the form of 'Anthem Money' that for use with Anthem-type services ($125.00 (individual), $250 (family) quarterly). The plans also pay 100% of 'Nationally Recognized Screenings/Tests for the member's age band' -- waiving the deductible. Pretty awesome. Here are some of their ballpark figures using a $2,500 deductible.
In addition, listed below are the approximate monthly premiums for a Lumenos HSA (that includes the wellness benefits mentioned above) with a $1500 deductible paying 100% (once the deductible is met):
